Position Summary

The Hospice Field Registered Nurse (RN) is responsible for providing skilled nursing care and case management to terminally ill patients in accordance with the hospice plan of care. The RN collaborates with the interdisciplinary team to ensure the highest quality of care, patient comfort, and support for families.

Key Responsibilities

  • Provide skilled nursing visits, assessments, and interventions in the patient’s home or facility setting.

  • Develop, implement, and update individualized patient care plans in collaboration with the interdisciplinary group (IDG).

  • Educate patients and families on disease process, pain and symptom management, and the hospice philosophy of care.

  • Administer medications and perform clinical procedures as prescribed.

  • Serve as the case manager for assigned patients, coordinating care with physicians, social workers, chaplains, and other team members.

  • Ensure accurate and timely documentation in compliance with agency, state, and federal regulations.

  • Participate in on-call rotation as needed to support patient needs outside regular hours.

  • Provide emotional support and guidance to patients and families during the end-of-life process.

Qualifications

  • Current Registered Nurse (RN) license in CA.

  • Bachelors of Science in Nursing 

  • Minimum of three years of nursing experience, hospice or palliative care preferred.

  • Strong clinical skills in pain and symptom management.

  • Excellent communication, organizational, and documentation skills.

  • Compassionate, patient-focused, and able to provide care in diverse settings.

  • Valid driver’s license, reliable transportation, and proof of insurance.

Compensation: $45.00 - $58.00 per hour
